To enable our doctors and staff to best meet your particular eye care needs, we ask that you complete our registration and health history forms before each annual exam.

These forms are available at the office and can be filled out at the time of your appointment. Many patients find it more convenient to download and complete these forms at home. This saves time in the waiting room and allows you the opportunity to double-check some of your background information. Please remember to bring your glasses to each appointment. If you are a new patient and wear contact lenses, it would be helpful for you to bring a copy of your prescription.

If you have gotten your eyes dilated recently, you know that this can be an interesting experience. Nearly all of our patients receive a dilated eye exam on an annual basis. Some patients find it difficult to drive in the bright sun after having their eyes dilated. As a precaution, we suggest that you plan to have someone available to drive you home. Also, when scheduling your annual exam, keep in mind that while your eyes are dilated, some visual tasks such as reading and computer use may be quite challenging for several hours following the exam.
